Missio Benefits Runout

This form is for members of Missio Benefits to indicate how they want to pay their portion of the runout, when to end ancillaries, and provide other details.

The estimated runout cost per employee is $2,038. For those converting with the rest of the group into the Guidestone plan, there is an estimated $738 reduction in the runout cost per enrolled employee.

We will incur most of the runout cost in the first few months of the program. It would help a great deal if you were able to pay this in one lump sum in October or November. However, we are offering 6 different payment schedules, reflected in the form below.

We will use the PlanSource system to do ACH pulls. This will be reflected on our PlanSource billing notification.
